 Customer Pick Up Date Request:
ALL DUMPSTER PRICING BASED UPON 10 CALENDAR DAY RENTAL. ALL DAYS THEREAFTER ARE CHARGED A $10/DAY RENTAL FEE. THIS CHARGE WILL BE COLLECTED UPON PICKING THE DUMPSTER UP. ALL PAYMENTS NEED TO BE CASH, CREDIT CARD, OR DEBIT CARD. IT IS THE CUSTOMERS RESPONSIBILITY TO CALL FOR DUMPSTER PICK UP. IF A FINAL PICKUP IS SCHEDULED, WE MUST BE NOTIFIED BY 6:00AM ON THE PICKUP DATE OF ANY CHANGES IN SCHEDULING, OR A $110.00 TRIP CHARGE WILL BE INCURRED BY THE CUSTOMER. THERE IS A 3% FUEL SURCHARGE ON ALL PRICING EXCEPT ON MUNICIPAL SOLID WASTE (MSW).

Trip Charge: If Elite Waste Disposal attempts to deliver or pick up a dumpster and is unable to perform the service requested, there will be a $115.00 trip charge (i.e.: no access to dumpster, brush in dumpster, etc.). If the driver needs to wait longer than 15 minutes on site, there will be a $115.00 Trip Charge + $80 per hour thereafter.
*City of Minneapolis/St. Paul Permit Fees:  If the dumpster placement is to be in the street in either the City of Minneapolis or St. Paul, Elite Waste Disposal must pull a permit with the city. The fees are as follows:
 City of Mpls: 30 Days: $95.75 ($65.75 +$30 admin fee)        City of Mpls: 60 Days: $161.50 ($131.50 +$30 admin fee)
 City of Mpls: 90 Days: $227.25 ($197.25 +$30 admin fee)   City of St. Paul: $5.00 per day + $30.00 Administrtion fee (3 day minimum)
* Elite delivers 6, 20 and 30 yard dumpsters. If you order a 10 yard, you are only allowed to fill the dumpster half full. If over half full, you will be charged the difference up to the 20 yard price.
Extra weight charges: If customer is filling the dumpster with a machine, debris is from a demo job or has any asphalt, concrete, brick, stucco, dirt, railroad ties, or timbers in the dumpster, there will be a charge for anything over:
Demo/Const $50/ton 1.0 to - 6 yard dumpster 1.5 ton- 10 yard dumpster 3.0 ton- 20 yard dumpster 4.5 ton- 30 yard dumpster
MSW $70/ton 1.0 to - 6 yard dumpster 1.5 ton- 10 yard dumpster 3.0 ton- 20 yard dumpster 4.5 ton- 30 yard dumpster
If any of the above items are going in the dumpster, a credit card number is required prior to the dumpster being dropped.

There is an additional $450.00 fee if there is any brush in a Demo/Construction or MSW dumpster, to be paid at the time of pick up.  NO STUMPS ALLOWED

The parties hereby mutually agree that an electronic signature is sufficient to bind the parties to this order and the agreement created thereby. By signing below it is my understanding that Elite Waste Disposal is not responsible for any of the damages resulting from the placement or removal of the dumpster at the above address. By signing this form this person has instructed Elite Waste Disposal on where to place the dumpster and he/she assumes all responsibility for damages.
